Dear Patients,

We would like to advise you of the Practice merger between Birchwood Medical Practice, Townhill Medical Practice and Smallfield Surgery. We have been working collaboratively since 2018 and this is the final formality of merging our clinical systems. These will begin to merge on Friday 13th October and we hope that the merge is fully complete by Monday 23rd October.

Our new overall Practice group name will be Modality East Surrey Medical Practice, but your GP Practice will still remain the same as before. You can still contact us on the usual telephone number and website.

Patients of these Practices will receive a letter in the coming few days with further details.
