We are proud to have been named finalists of Primary Care Network of the Year on Friday 5th December at the General Practice Awards 2025. Serving 45,000 residents, our network has worked tirelessly to build a mature, integrated model rooted in the wider determinants of health and the voices of our community.

Recognised for our commitment to prevention, integration and community partnerships, Healthy Horley PCN continues to demonstrate what neighbourhood health creation can achieve.

The Horley Health Hub, now nationally recognised, is at the centre of this approach. It has become a trusted non-clinical space where partners, volunteers and residents come together for bereavement support, men’s wellbeing groups, gardening clubs, community events and prevention programmes – easing pressure on general practice while strengthening connection and resilience.

Alongside this, the PCN delivers a suite of integrated services, including:

  • Frailty and anticipatory care, supporting patients at home and preventing avoidable admissions.
  • Mental health support, through GPimhs and voluntary sector partners.
  • Employment support, delivered through IPS Workwise specialists to address the wider determinants of health.
  • Community groups and volunteer-led initiatives, powered by Health Champions and Growing Health Together.
  • Peer-supported wellbeing and prevention programmes that reduce isolation and improve engagement.

This award celebrates a PCN that listens, collaborates and designs solutions with its community. We are proud to stand as an example of what integrated neighbourhood care can achieve when partnerships, prevention and people sit at the heart of the model.

Congratulations to Modality East Surrey, part of our PCN, for winning General Practice Team of the Year – a fantastic achievement that reflects the strength of our shared vision for integrated care.
